Q: is the large size just longer than the medium or is it also wider? I need it to fit snugly but also be long enough to cover the entire big toe.

The large size is what I bought, so I have no knowledge about other sizes. However, I wear a size 11 shoe (women's) with a fairly big toe, and the gel protector fits beautifully - nice & snug and just the right length.. No cutting needed. In fact, I forgot I had the protector on the first day I wore it, which indicates how comfortable it is on the toe.

Q: Are these just for the big toe? Can I use on middle toe?

They are just for the middle toe. There is one photo that shows it on a middle toe but it is inaccurate. I found them worthless because they are all the same size and so would not fit on my middle toe. There is another brand on Amazon that works much better for the middle toe.
