Q: How high does the thumb rest sit above the face of the bass? Is it lower than the E string (similar to a pickup) allowing clearance?

If you install the thumb rest in the manner specified by the instructions you should have proper string clearance and no issues. I would say it's definitely taller than the p-bass pickup though.

Q: Can someone please tell me the screw size & length? I purchased and then promptly and foolishly lost both screws.

It's about 1 5/8" x 1/2". I mounted mine by putting a layer of duct tape to the bottom (sticky to the thumbrest) of the thumbrest for friction and used 1 of the pickguard screws to secure it. I did that for removal if I didn't like it. It worked and I'll keep it that way until it loosens. Had for about 6 months and is working fine.

Q: Does this fit the pre-drilled holes on a fender road worn 50s p bass?

These have a center-to-center spacing of 1.25"(31.8mm). They should work but I would measure your existing holes to insure a proper fit.
