Has anyone been able to adapt the thumb portion so it better fits the thumb? If so, I'd really appreciate hearing how to. Thx so much for any help!

The entire surface is stick-on fabric,you can attach the strap wherever you feel comfortable. If you feel the strap is not holding your thumb tightly,you can attach the strap closer to the underside of your thumb rather than wrapping it around your thumb. This will help keep your thumb in place and help prevent thumb movement from slipping out of the strap. You can refer to the schematic diagram on our details page.

I need to stretch my thumb away from my index finger with firm hold. And need it to be adjustable as i gain motion. Will this work?

I think so. The thumb has a loop made of stretchy fabric plus a separate velcro over strap that goes around the outside of that loop so you could adjust the thumb position to where ever you want by placing something as a spacer (e.g.: soft sponge, fabric roll, etc.) under the thumb pad. The base plate is rigid but all digit loops are flexible like an ace bandage. Hope that helps.
