how cold does this get?

The literature says 18-20degrees C below the ambient temperature. I can't confirm this as I don't have a thermometer but it gets cold enough to store dairy produce eg after 2 days milk is still completely fresh and as cold as it would be in my fridge/freezer. Haven't tried meat but I think a meat snack would be fine

Is the temperature same as usual large fridges 4 degrees or below? Need to ensure its cold enough to store expressed milk.

I can't give you a definitive answer. What I can say is that it keeps pasteurised and sterilised milk cold enough without going off.
