Does this come with the type 3 cable for the HX stomp?

It does not. You have to buy both the voltage doubler and the yellow-tipped HX Stomp sized cable separately. Neither were available on Amazon last I checked, but you can get them at Sweetwater or the Eventide online store.

Is it sturdy enough to be used on top of pedalboard as a pedal riser?

It is very sturdy so I think you can. Keep in mind it gets a bit warm so you should use good adhesive on your pedal

Will this provide the full 660ma on all outlets simultaneously? if not, what's the maximum load for the psu total.

660mA at 9V on each outlet. I’ve been very pleased with mine. Pairs great with a Temple pedalboard
