This is the real deal
This Syringe is a heavy duty syringe very similar to the ones used in the doctor's offices. Pryor to seeing this Syringe I had ordered one half the price that was virtually worthless but this one is the real deal. Don't waste your money on a cheap plastic one when you can get this one.The only negative was that it came with no directions. Sure the use is pretty evident but it wouldn't hurt to have a page talking about any hazards of use. It comes with two different tips but I have only used the rounded tip and would be a bit concerned using the more pointed tip. I do think you have to use care with either tip. Here are a couple of links with information on cleaning ears. I do not endorse any of these and you should decide if you wish to follow them at your own accord and risk:[...][...]

Performs equal to doctor's office instrument
I have had to irrigate my ears for years due to excessive wax build up. The cost to go to a doctor for this has become prohibitive, so I ordered this syringe and my wife administers it to me. The key to success with this device is using some type of over-the-counter ear drops (wax softner) or making your own solution (found on the Internet) for several days prior to irrigating your ears. Also, be careful not to over-irrigate. In other words if you do not get all the wax out after several squirts, use more ear drops and try again in a day or two. It is also very helpful to invest in an led otoscope (can be purchased on Amazon; Dr. Mom is a good brand).

It's not air tight at the top so it only sucked in maybe 10cc of the 4 oz capacity
This arrived with the packaging already opened. The plastic sacks for the different components were all broken open. It had a strange grease all over the parts. Finally, only one of the two tips screwed into the syringe. The other was to long so it wouldn't go in all the way. Also it doesn't work, even with the properly fitting tip. It's not air tight at the top so it only sucked in maybe 10cc of the 4 oz capacity. No doubt this was a defective return, so I'm sure it will be resent to some other unsuspecting customer.

Looks well made. The inside gunk is lube and is required.
I was a little worried when I ordered this because of some of the reviews mentioning it was nasty on the inside and even possibly used. Well, that inside goop is lubricant that is needed to provide proper suction. This unit uses a rubber O-ring to make a seal for the piston and that requires a lubricant. The manufacturer wanted to make sure it did not dry out so they put a bit more than needed and that's a good thing. If it bothers you just wipe it out but be sure to leave some on the rubber O-ring or it will not work well at all. It's too bad that some people post negatives for an issue that in reality is a positive.

Needs some Improvement
Started out really liking it but after using it a few times and letting it set for about a month the piston froze up. I couldn't use it. I Took it apart and discovered it (the piston) was made from aluminum which had corroded and oxidized to the point that it was stuck in the barrel. There also was no lubricant on the piston to help things slide and seal better. I work on industrial hydraulic equipment and would never dream of assembling something like this. The piston should be made from a material that won't react with ordinary water such as brass, stainless steel or a plastic and assembled with a water proof lubricant such as silicon grease. Another problem is the poor quality threads on the nozzles. They are rough and seize when screwing into the body. If these two problems were resolved it would be a super tool for the price. I have to conclude that this not the same one used by your doctor. Doctors would never tolerate this poor quality. If you buy this I would recommend you disassemble it and coat the piston and O rings with a waterproof lube such as Vaseline or silicone grease. It might work well for you.

Good, but not great.
My ears get a lot of ear wax build up in the humidity and when they get water in them it doesn't come back out and so they get swollen and sore. My kids are the same, but worse. I bought this to flush them out more frequently than the unwanted, inconvenient trip to the doctor to get that done. The item came with adequate packaging, which not much is really needed for an item made of metal. It is sturdy so I'm not worried about it breaking. The quality of the metal is questionable though, and feels like it's grinding when you unscrew it. The end with the plunger is hard to get back on when you take it off because of the threads. I accidentally cross threaded it but didn't seem to ruin the threads because I can still get it on properly and easy enough. It had some questionable lubricant inside it when I got mine. I took it apart right away because of the other reviews I read before purchasing. I'm not sure what was in it, but my concern wasn't for the lubricant that was used because it was gritty and definitely shouldn't have been there. It seemed like there was some particles left from the manufacturing process and not cleaned properly prior to lubrication. I cleaned it with hot/warm soapy water and re-greased it with vaseline which was suggested from another reviewer. I'm a little skeptical about using vaseline since it's a petroleum based lubricant and can ruin some rubber materials but we'll see. After getting it back together it worked well. It wasn't a perfect seal because pulling the plunger back, after pushing the water out, a little bit of the water squirted out the wrong end. The inside also had some defects which I'm assuming is not giving it a tight seal. Inside it looks like it was against a grinder for a half second and made a couple of grooves. It is definitely not unusable though. There was also a flat plastic string under the O-Ring which was sticking out just a bit so I pulled on it not sure what it was. I'm assuming this was used to make a better seal against the syringe too. I am going to see if I can get a slightly bigger O-Ring and replace this one to see if I can get a better seal without it being too tight to plunge. Might have to settle. It also comes with 2 tips; one for inside the ear to flush the canal and one that can't go in the ear which you can use to clean the outer part of the canal. I would have given this a better rating if it weren't for the mess inside and having to "hopefully" find a better solution to the problem with the seal.
