I had no problem keeping it in place when I slept with my knees stacked, even without using the elastic strap. But as a side sleeper I don't really spend much time with my knees stacked, I prefer to bend one knee towards my chest and straighten the other leg. This pillow doesn't work so well for that. I purchased a soft pea shaped pillow is more comfortable for the way I sleep.
