Exactly as described. A great replacement by Toyota that should be on the truck from manufacture and not an aftermarket. Throw away the plastic cap and buy this replacement. The added wrench works fine as well. Save yourself the headache and just get this replacement.

OEM Fits fine, only time will tell
Toyota was trying to be environmentally conscientious by making this style filter/filter housing cap. Good for the environment, bad for the DIY consumer, especially when the housing is made from a plastic polymer. This one is made from aluminum and appears to be more sturdy than the factory one.

I have yet to use this as I'm not quit do for service. However, all parts that are displayed are exactly what you get. I am really impressed with the quality.
