Q: Is the bottom placement on the seat of the chari or car, or is it positioned up higher?

It's adjustable along the back of the chair. You can place it on the seat or up higher as it feels comfortable. The strap doesn't hold it too tightly though which is my biggest complaint

Q: Is this just a cover or does it come with the pillow as well?

This is a very good pillow WITH a removable cover. Really, I like the idea of a removable cover, rather than having to throw the whole pillow in the wash:

Q: Will this cushion fit a standard folding chair, the type with padded seat and open lower back? Or will it fall thru the opening?

I suppose the lumbar cushion will fall through the opening, there is a strap on the lumbar, it may help to fix it on the folding chair but it may not work very well as the lumbar will slip away or slip out then your back lean against on it. Q: Which side goes bottom/top?

Whatever feels comfortable to you. The 'correct' way is to have the narrow end up. I've used it the other way and it fits fine depending on the chair.
