May need to be reset with LSIUtils before it can work
1st, the seller has a lousy shipping process. They sent shipping info to USPS right away, but after a week, USPS had not received the actual shipment and flagged it as undeliverable. a couple days after that, after I had given up hope, USPS finally received it, and shipped it out, but Amazon still thinks it's undeliverable.2nd. the product (not new obviously) had previously been used in some specialized server environment which did not use the LSI/Broadcom firmware. All the actual drive ports on the card had been disabled and the card was unusable and not detected as having any drives attached. I have another LSI card with the LSI bios on it that [should have] detected the card, but it was not seeing it either. After a couple of days of trying fruitlessly to reflash it and pulling my hair out, I luckily came across a u-tube video about resetting the LSI cards that are shipped from lazy shippers who did not bother to reset them to factory status after they were pulled out of the specialized servers they had been reconfigured for. After creating a DOS boot disk, and dumping the LSIUtils utility on it, I was able to boot up and run the utility to reset the card to factory defaults, after which it detected all the drives, and showed up when booting, etc. So all is good now.But a warning to any potential buyers, these cards are all older, used cards which were likely pulled from enterprise servers which used specialized hardware to access them, and may need to be reset before they are functional in a normal environment.The card itself (LSI 9201-16e) is great for plugging in a big stack of spinning drives that you want to set up with sofware raids. The 6Gbs speed is plenty fast for spinning drives which top out at 190-200Mbs individually. If you are setting up with a stack of SSD you may be better off looking into a 12gbs card. (I assume you have a stack of drives to hook up if you need a 16 port sas card)

Works Fine in unRAID
Purchased this in April 2018 and was sold by Made PC shipped by Amazon. The card looked brand new when I received it although it just came enclosed in an ESD bag covered in bubble wrap. The revision I received was H3-25577-00A. I don't know what the difference is between the different revisions of this card you see floating around. All I know is I needed to upgrade my 8 port SUPERMICRO AOC-SASLP-MV8 card in my unRAID system so I could add more drives with an external drive enclosure. This LSI card was a drop in replacement for the unRAID system and was less than half the price I originally paid for the Supermicro card many years ago. After swapping out the cards unRAID booted up fine and I didn't need to change any settings to make things work. I bought a Dual Mini SAS SFF-8088 To SFF-8087 Adapter which installed into a spare PCI slot back plate. Then I connected 2 8088-to-8088 cables out of this card into the adapter. Then I connected my existing 8087 SAS-to-SATA cables inside the case to the adapter. Everything works fine. I'll next hook a cable from the LSI card to another 8088-to-8087 adapter I have in another computer case that has my additional hard drives.

Model shipped not model sold.
Sold as model H5-25379-00. Model H3-25577-00A arrived. More or less same product. Only reason this is a 2 star review.Only reason I'm not returning is the price. They're the same chip-set, so it'll do the job. They're the same card, more or less, but when you sell a specific model, you dang well better ship that model.
