Q: I'm 5'8" 130 pounds....32 inch bust kind of narrow shoulders. I know it says medium is my size but would small fit better?

If your frame is that small I would get a small. You don't want it too big cuz then you won't get the support you need. I'm way shorter than you 5'2 bust 34 119 lbs ..small is perfect. .. it should fit snug

Q: Is there a big difference between the small and the extra small?

Yes the xs is for kids or people who fit the size criteria on the sizing chart. If you are an adult with very tiny shoulders then this may work for you.

Hi,In the search bar on Amazon, type Posturemedic and click on the 1st link. When you do it will pull up the PostureMedic pic of a girl wearing it. To the left of that pic, you should see 5 small pictures, click on the colorful picture and that it the size guide.
