Mine has no markings as to where it is made. There are two imbossed patent numbers and a recycle "2" molded in, this thing is so strong and sturdy, I'm leaving it to my nephew in my will as it will outlast me in 10 life times. Never ever seen a creeper this strong and balanced.

Will creeper work ok on an asphalt driveway? Are wheels made of rubber, or a very hard material?

I think they are hard plastic. I used it on my concrete driveway, but if you have any cracks in your asphalt you would want one with larger wheels.

Can this creeper be used surfaces consisting of dirt, gravel or small rocks?

This creeper has small wheels, and little clearance, it works best on hard surfaces. You will not be happy off-road.

How many inches off the ground are the top surfaces where your head and body rests?

I apologize for the delay. Since I seldom use this creeper, I misplaced it and it took awhile to find it which I finally did. It measures 6 inches from the ground to the top of the headrest and 4 inches from the ground to each wheel base. I now use a kitchen mat which is cushioned and a firm oval square pillow. Believe me, it's much more comfortable than lying on a hard creeper for hours, especially when you're a senior citizen.
