Why is it good his book?Well, starts for defining errors and explaining how they are a normal part of every working system. The explanation of the type of errors amplÃes the ones that we have always supposed we commit as humans. We are that, humans, working in auto mode or not. He doesn’t go that path on how to avoid them, but just explaining them. And that’s the beauty part, basic definitions. Can we understand how to avoid them? I believe we will have to read the next book where he explain the system per se.Thanks for this master piece

Great book to understand an academic yet practical take on human error. Thoroughly, enjoyed reading this book; a must read for anyone who is planning to pursue a career in Human Factors.

A fabulous book describing and summarising some of James Reasons thoughts on Human Fallibility. This works both as an introduction to the topic or a review for those who have some knowledge in the area

Like a course syllabus for a human factors course. A great outline of major points within a personal, human perspective.

Disappointed
I was expecting more. Reason is a brilliant person. By virtue of the title as well as the clever cover, I was expecting more in the way of illustrative error examples, rather than the more scholarly expository of his theoretical error models. Instead, he gives only a few error events, and then leaps into rather sophisticated perceptual and behavioral discussions.

A pamphlet
I thought this was an actual book, but actually it is a drawn out leaflet of material easily findable (and in better form) on many websites. Highly whatever.
