What is the diameter of the anvil? I intend on purchasing a ball head attachment, and Mitutoyo offers 0.25" and 0.2" attachments.

If you took the time to look at documentation for the Micrometer from the Mitutoyo web site, you would find that the spindle lock diameter and anvil are specified at 6.3mm which in English Units maps to .2480"

Is it supposed to come with the standard bar?

This item does not come with a standard. It comes with the micrometer, battery, plastic case and sleeve.

Can you use this to measure the thickness of paper?

The thickness of paper is 0.004 +/- and this micrometer accurate to 0.0005 with no problem.

There's no standard bar in the case. Yet the included manual refers to a "supplied standard bar". Why does the manual mention that bar as "supplied"?

The manual is probably written for all of the micrometers including the ones over 1" which would need a standard bar. The 0-1" doesn't need a bar because it is checked at physical zero point- that is, with the spindle actually touching the anvil.
