Can the lever head be mounted on a flat surface and have the lever still work or will it need the base mount too?

It will need the base mount. the lever itself is not stand-alone.

Can you attach a gopro adapter to the bottom of this, so it can be attached to a gopro mount?

Hi, I am not 100% sure if this will work with the device you have. Most of Quadlock are compatible with Go Pro therefore I assume it will. There is free return service offered by FBA if the item you purchase is incompatible. Thanks
